Minnesota Business Assistance Form (MBAF)
The Minnesota Business Assistance Form (MBAF) should be used to report and/or update each calendar year's activity by April 1 of the following year. Due to the delay in finalizing the MBAF revision, the deadline this year for the form submission has been extended one month to May 1, 2008.
- JOBZ MBAF [PDF] - Use this form to report only JOBZ project activity from January 1, 2004, through December 31, 2007.
- Non-JOBZ MBAF [PDF] - Use this form to report only non-JOBZ project activity from August 1, 1999, through December 31, 2007. Also available is the online Non-JOBZ Minnesota Business Assistance Form.
