Community Rehabilitation Programs
January 1, 2006, Rehabilitation Services implemented an outcome-based system for placement services funded by the Vocational Rehabilitation Program. This change will impact how VR counselors authorize services and how service vendors report and are paid for the provision of those services. The following information is intended to assist all affected Community Rehabilitation Programs (CRPs) and Limited-Use Vendors (LUVs) make the transition to this new system.
CRP PROVIDERS: A list of all CARF-accredited CRPs with whom Vocational Rehabilitation Services has an Operating Agreement and Fee Statement. "Informed Choice" and "Summaries of Services Purchased" dates for each CRP are provided.
CRP FORMS and INSTRUCTIONS: For processing new or revised Operating Agreements, Fee Statements, and ADA Reviews.
LUV POLICY and FORMS: For reference and use by Limited-Use Vendors.
LUV PROVIDERS: A list of LUVs with whom Vocational Rehabilitation Services contracts (currently unavailable).
CRP REPORTS: "Comparison Reports" indicating the number of consumers served and amounts of funds authorized by Vocational Rehabilitation Services for each CRP, per federal fiscal year.
PERFORMANCE-BASED SERVICE DELIVERY SYSTEM: Referral process, outcomes, services affected by PBA, Frequently Asked Questions, contracting requirements, becoming a service provider, CARF Accreditation, vendor status (CRP or LUV), PBA Workgroup, etc.
