SFY 2009 Application for Funding of Certified Extended Employment Providers
Certified Extended Employment Program providers shall apply for funding before Rehabilitation Services will establish contracts for ongoing employment support services.

The forms need to be opened in Microsoft Word or Excel. The instructions are a guide to completing the application and will provide information about funding this year.
Complete the forms, print, obtain original signatures, and mail or deliver to Attention Wendy Keller :

Complete Hardship Waiver Requests shall be postmarked by September 24, 2008 , or received by Rehabilitation Services no later than 4:30 pm, September 24, 2008, to be considered for funding.
